Holiday Party Online Payment Option
Online payment option for our 2015 Holiday Party at Maggiano's Little Italy in Buckhead.
Price per adult: $ 25
Price per child (12 and under): $15
Reservations and payment due by December 7, 2015 so we can give a count to the restaurant.
Please note: a convenience fee of paying online of $1.50 will be added and described as "Shipping Charge"
Once you have paid online, you will receive from Paypal an email with a transaction ID. Please write this transaction ID on your reservation form and either:
a) scan and email the form to Claire Ellis at: claire620@hotmail.com
b) mail the form to Claire Ellis, CKCSCGA Treasurer at : 3949 Saint Elisabeth Square, Duluth Georgia 30096-8049

1. To pay via check:
- Print out the reservation form and complete
- Make check payable to "CKCSCGA"
- Mail reservation form and check to:

2. To pay via paypal using our online store:
- Make your online selections and complete transaction (please note there is a $1.50 convenience fee added to the transactions).
- You will receive an email from Paypal with a transaction id number for the payment you have made to CKCSCGA
- Print out the reservation form and complete and write down the paypal transaction id number on the reservation in the spot indicated.
- Scan and email the form to Claire Ellis at: claire620@hotmail.com with subject line: CKCSCGA Holiday Party Reservation
